These Terms and Conditions were last updated on June 2026 and replace all prior versions.
By submitting a booking request through our website, you confirm that you have read, understood, and agreed
to each section below.



1. Introduction

These Appointment Booking Terms and Conditions (the “Terms”) govern all booking requests
submitted to SMILE FOR PET LLC (“Smile4Pet”, “we”,
“us”, or “our”) through our website at
smile4pet.com, by email, by phone, by text message,
or by any other method we accept.

By submitting a booking request, you (“Client”, “you”, or
“your”) acknowledge that you have read these Terms and agree to be bound by them.
If you do not agree, please do not submit a booking request.

These Terms constitute a legally binding agreement between you and SMILE FOR PET LLC,
a limited liability company organized under the laws of the State of Connecticut.

1.1 Authority to Book and Consent

By submitting a booking request, you confirm that you are at least 18 years old and that you are either
the legal owner of the pet or have full authority from the pet’s owner to schedule the appointment,
provide information about the pet, accept these Terms, authorize the service, and assume responsibility
for all charges and obligations related to the appointment.

If you book on behalf of another person, family member, friend, business, rescue, shelter, or any other
third party, you are responsible for making sure that the actual owner or authorized decision-maker has
reviewed and accepted these Terms before the appointment takes place.



2. Booking and Confirmation

2.1 Request vs. Confirmed Appointment

Submitting a booking request through our website or by any other method does not constitute
a confirmed appointment. All requests are subject to review and explicit approval by Smile4Pet. Your
appointment is confirmed only when you receive written confirmation from us by email, text message,
or another written method we use for scheduling.

2.2 Confirmation Process

After submitting your request, our team will review your information and respond within
1–2 business days when possible. We may contact you directly if we have questions,
need additional records, need photographs of your pet’s mouth, or need to discuss scheduling.
No appointment slot is held or guaranteed until written confirmation is issued.

2.3 Appointment Reminders

We may send appointment reminders by email and/or text message before your scheduled visit.
Please reply when confirmation is requested. Failure to confirm, failure to provide required information,
or failure to provide required vaccination records may result in the appointment being released, declined,
or rescheduled.

2.4 Accuracy of Booking Information

You are responsible for providing complete and accurate information during the booking process, including
your contact information, service address, pet information, vaccination status, health history, behavior
history, and any other details reasonably requested by Smile4Pet. We may rely on the information you provide
when deciding whether to accept the appointment and whether the service can be safely performed.



3. Cancellation Policy

3.1 Cancellation Notice Requirement

Cancellations must be made at least 24 hours before the scheduled appointment time.

3.2 Late Cancellations and No-Shows

Cancellations made less than 24 hours before the appointment, failure to be present at the agreed service
address at the scheduled time, failure to make the pet reasonably available, or failure to provide required
records or information may be treated as a late cancellation or no-show. A late cancellation,
no-show, travel, or administrative fee may apply if it was disclosed during booking confirmation or in other
written appointment communications.

3.3 Cancellation by Smile4Pet

We reserve the right to cancel or reschedule any appointment due to circumstances beyond our control,
including but not limited to severe weather, road conditions, vehicle issues, technician illness, emergency
situations, safety concerns, incomplete records, or concerns about the pet’s health or behavior. In such cases,
we will notify you as promptly as possible and, when appropriate, offer an alternative appointment or a refund
of any pre-paid fees.



4. Service Charges and Billing

4.1 Service Pricing

All pricing is provided in US Dollars (USD). Prices may vary by service area, travel requirements,
number of pets, appointment type, clinic or partner-location arrangements, and any special circumstances
disclosed before confirmation. Prices are subject to applicable state and local sales tax where required by law.

4.2 Price Accuracy and Changes

We make every effort to ensure that prices communicated to clients are accurate and current. However,
we reserve the right to correct pricing errors and to update our service fees at any time. Any price change
will not affect a booking that has already been confirmed in writing unless the original price was clearly
incorrect or based on inaccurate information provided during booking.

4.3 Deposits, Pre-Authorization, and Advance Charges

We may require a deposit, advance payment, or payment pre-authorization to secure an appointment
slot. Any such requirement will be communicated before or at the time of booking confirmation. Deposit and
refund rules may vary by appointment type, service area, clinic or partner-location appointment, and cancellation
timing.



5. Payment Terms

5.1 Payment Due Date

Unless otherwise agreed in writing, payment in full is due at the time of service, upon completion of the
appointment. We do not extend credit or deferred payment terms unless explicitly arranged in advance.

5.2 Accepted Payment Methods

We accept the following forms of payment unless otherwise stated in the appointment confirmation:

  • Cash
  • Personal or business checks — accepted in Connecticut only; checks must be made payable to Smile For Pet LLC
  • Credit and debit cards
  • Apple Pay, Android Pay, and Samsung Pay
  • PayPal — payments via paypal.com; standard PayPal transfer fees may apply
  • Zelle — direct bank transfer via the Zelle network; transfers are typically instant and irrevocable once sent

5.3 Non-Accepted Payment Methods

We do not accept Venmo, Cash App, cryptocurrency, or any payment method not listed in
Section 5.2. Payments made through non-approved platforms cannot be guaranteed to be received and will
not be treated as settlement of your balance unless we confirm receipt.

5.4 Chargebacks and Payment Disputes

Initiating a chargeback or payment dispute for services that were duly rendered may be treated as a breach
of these Terms. We reserve the right to pursue recovery of unpaid or disputed amounts, including applicable
processing fees, collection costs, and reasonable legal fees where permitted by law.



6. Service Location and Requirements

6.1 House-Call Service

Smile4Pet provides mobile, in-home, anesthesia-free oral hygiene services for pets. Our technician travels
to your home or another agreed service address within our service area. All house-call services are performed
on-site at the Client’s location. Smile4Pet does not operate a fixed public salon or public grooming facility.

6.2 Service Area

Smile4Pet currently serves selected areas in Connecticut, New York, Manhattan, and Rhode Island. Service
availability is verified during the booking process and may depend on ZIP code, technician routing,
appointment type, clinic or partner-location availability, and scheduling capacity.

Our Connecticut service area includes Hartford County, Middlesex County,
New Haven County, and New London County. Fairfield County
is partially covered. In New York, Westchester Countyis partially covered. In New York City,
Manhattan is partially covered. In Rhode Island, service is limited to selected areas of
Washington County. We do not guarantee service to every town, ZIP code, building, address,
or neighborhood within a listed county or area.

6.3 Veterinary Clinics and Partner Facilities

Smile4Pet may, from time to time, offer appointments at veterinary clinics, animal hospitals, grooming salons,
pet care businesses, or other approved partner facilities. These appointments are available only when specifically
offered and confirmed by Smile4Pet.

Unless otherwise stated in writing, the partner facility provides the location only. Smile4Pet remains responsible
for performing the anesthesia-free oral hygiene service through its own technician. A veterinary clinic or partner
facility does not supervise, direct, guarantee, or assume responsibility for Smile4Pet’s service unless a separate
written arrangement says otherwise.

Clinic and partner-facility appointments may be subject to additional rules, forms, arrival times, vaccination
requirements, check-in procedures, cancellation rules, parking rules, or facility requirements. Veterinary clinics
may need to prioritize medical emergencies or hospital operations, which may result in delays, rescheduling, or
cancellation. By booking a clinic or partner-facility appointment, you agree to follow the reasonable instructions
of both Smile4Pet and the facility.

6.4 Client Responsibilities at the Service Location

To ensure a safe, effective, and comfortable appointment for your pet and our staff, you agree to:

  • Provide safe and reasonable access to your home or service address at the scheduled appointment time.
  • Ensure a clean, clear, quiet, indoor working area where our technician can work without obstruction.
  • Provide access to an electrical outlet and reasonable lighting.
  • Have two clean towels available for the technician’s use if requested.
  • Make sure your pet has had a short walk or reasonable opportunity to relax before the appointment.
  • Do not feed your pet for at least 1 hour before the appointment unless your veterinarian has instructed otherwise.
  • Ensure that other pets and children, below the age of 14 years, at the premises are safely secured and away from the working area during the procedure.
  • Disclose any known behavioral concerns, including aggression, biting, severe anxiety, sensitivity to handling, prior restraint difficulty, or need for a muzzle.

6.5 Right to Refuse, Pause, or Discontinue Service

Smile4Pet reserves the right to refuse, pause, or discontinue a service at any time if, in our technician’s
professional judgment, proceeding would pose a risk to the safety or well-being of the pet, our staff, the Client,
the facility, or any other person present. The technician’s safety and candidacy assessment is final.

If service is refused, paused, or discontinued because of undisclosed health issues, undisclosed behavior issues,
unsafe conditions, missing records, lack of access, or any other condition outside Smile4Pet’s reasonable control,
a partial charge, travel charge, clinic fee, or cancellation fee may apply.



7.1 Transactional Communications

By submitting a booking request and providing your email address and/or phone number, you agree to receive
appointment-related communications from Smile4Pet related to your request, including booking questions,
confirmation messages, appointment reminders, vaccination-record requests, photo requests, status updates,
payment-related messages, and follow-up messages regarding your specific appointment.

These communications are necessary to process and provide the requested service. If you do not want to receive
appointment-related communications, we may be unable to schedule or perform the appointment.

7.2 Marketing and Promotional Communications

Smile4Pet may send marketing or promotional communications only where permitted by law and, where required,
after receiving appropriate consent. Marketing messages may include pet care information, service updates,
seasonal promotions, special offers, or announcements. You may ask us to stop sending marketing messages at any time.

7.3 Text Messages

Smile4Pet uses ordinary text messaging to communicate with clients about appointments instead of relying only on
phone calls. We may also use third-party platforms, including Square and Brevo, to send appointment reminders, payment links,
receipts, or other transactional notifications.

If you no longer wish to receive ordinary text messages directly from Smile4Pet, please tell us by replying to our
message or contacting us at info@smile4pet.com or
(929) 456-1020.

Some automated notifications may be sent separately through Square or another third-party platform. If a third-party
system provides its own opt-out instructions, such as replying STOP, those instructions apply to messages sent through
that platform and may not control ordinary direct text messages from Smile4Pet.

7.4 Consent Is Not a Condition of Purchase

Your consent to receive marketing communications is not required as a condition of booking or receiving
services from Smile4Pet. Appointment-related communications may still be required to schedule, confirm, perform,
bill, or follow up on a requested appointment.



8. Privacy and Data Protection

8.1 Information We Collect

When you submit a booking request, we collect the personal information you provide, including your name,
phone number, email address, service address, and information about your pet(s), including vaccination records,
health history, behavior information, and photographs of your pet’s teeth, gums, or mouth when requested.
This information is used to process your booking, determine whether the pet may be a candidate for service,
deliver our services, communicate with you, maintain business records, and comply with applicable laws.

8.2 Photographs and Service Records

Smile4Pet may take or request photographs of your pet’s teeth, gums, mouth, or visible oral condition for booking
review, service records, before-and-after comparison, quality control, technician documentation, and client communication.
We will not intentionally use photographs that clearly identify you, your home, or private personal information for
public marketing without additional permission.

8.3 Your Privacy Rights (Connecticut Residents)

If you are a resident of Connecticut, you may have rights under the
Connecticut Data Privacy Act (CTDPA), including the right to:

  • Access the personal data we hold about you;
  • Correct inaccurate personal data;
  • Delete your personal data, subject to certain legal exceptions;
  • Obtain a portable copy of your personal data in a readily usable format;
  • Opt out of any sale of your personal data or its use for targeted advertising (we do not sell personal data or use it for targeted advertising).

We will respond within 45 days. We will not discriminate against you
for exercising your privacy rights.

8.4 New York Residents — SHIELD Act

If you are a resident of New York State, your personal information is protected under the
New York SHIELD Act. We maintain reasonable administrative, technical, and physical
safeguards appropriate to the size and nature of our business to protect your data from unauthorized
access, use, or disclosure. In the event of a data breach involving your personal information, we will
notify you within 30 days of discovery, as required by New York law.

8.5 Third-Party Service Providers

We use third-party platforms to operate our business, including payment processors, scheduling tools, communication
tools, email platforms, cloud storage, website tools, and customer-management systems. These providers may process
information on our behalf. We do not sell your personal information.

8.6 Data Retention

We retain booking records, service records, payment records, communications, photographs, vaccination records, and
related information for as long as reasonably necessary to provide our services, maintain business records, comply
with applicable laws, resolve disputes, and protect the rights and safety of Smile4Pet, our staff, our clients,
partner facilities, and pets.

8.7 Privacy Requests

You may contact us at info@smile4pet.com to request
access to, correction of, or deletion of personal information associated with your booking. Some information may need
to be retained where permitted or required by law, for business records, dispute resolution, fraud prevention, or
safety documentation.



9. Pet Health, Safety, and Candidacy

9.1 Health Disclosure Requirement

You are required to disclose any known or suspected health conditions, recent surgeries, medications,
allergies, pregnancy, seizures, heart conditions, respiratory conditions, collapsing trachea, fainting episodes,
severe anxiety, oral pain, bleeding, infection, loose teeth, recent dental procedures, prior anesthesia complications,
infectious disease, or any other medical or behavioral issue that may affect your pet’s ability to safely receive
an anesthesia-free oral hygiene service.

Failure to disclose complete and accurate information may result in refusal or discontinuation of service and releases
Smile4Pet from responsibility for outcomes related to undisclosed or inaccurately disclosed conditions.

9.2 Vaccination Requirements

For the health and safety of your pet, other pets, our staff, clients, and partner facilities, we require proof of
current vaccination before an appointment is confirmed or before service is performed. Dogs must be current on
Rabies and DAP/DHPP. Cats must be current on Rabies and applicable
core vaccinations when requested. We reserve the right to decline or reschedule service if vaccination records are
missing, unclear, expired, incomplete, or not provided within the required timeframe.

9.3 Scope of Service

Smile4Pet provides an anesthesia-free oral hygiene service for dogs and cats. Our service is not
veterinary medicine, is not a veterinary dental procedure, and is not a substitute for examination, diagnosis,
dental radiographs, anesthesia-based dental cleaning, periodontal treatment, extractions, oral surgery, or any other
veterinary care.

Smile4Pet does not diagnose, treat, or manage dental disease or other medical conditions. If your pet has pain,
swelling, bleeding, loose teeth, broken teeth, infection, severe periodontal disease, oral masses, or any condition
requiring medical judgment, you should consult your pet’s local veterinarian service.

9.4 No Guarantee of Results

Results vary by pet and depend on factors such as temperament, level of tartar, oral anatomy, gum condition, age,
cooperation, medical status, and the amount of work that can be safely completed without anesthesia. Smile4Pet does
not guarantee that all tartar, staining, odor, or visible buildup can be removed, and we do not guarantee any medical
or dental outcome.

9.5 Emergency and Veterinary Care

Smile4Pet is not an emergency veterinary provider. If your pet experiences a medical emergency before, during, or
after an appointment, you are responsible for seeking veterinary care immediately. You are responsible for all veterinary
costs, emergency costs, medications, diagnostics, treatment, transportation, and follow-up care for your pet, except
where prohibited by law.



10. Liability and Limitation of Responsibility

10.1 Limitation of Responsibility

To the fullest extent permitted by law, SMILE FOR PET LLC is not responsible for any adverse
reaction, injury, illness, worsening of condition, stress-related reaction, behavioral reaction, or other outcome
related to:

  • A pre-existing, underlying, undisclosed, or inaccurately disclosed medical condition.
  • A behavioral condition, aggression, anxiety, fear response, restraint difficulty, or unpredictable behavior.
  • Anatomical abnormality, age-related condition, respiratory condition, heart condition, seizure history, oral pain, dental disease, infection, loose teeth, or other condition outside the scope of our service.
  • Incomplete, inaccurate, outdated, missing, or misleading information provided by the Client or a third party.
  • The Client’s failure to follow preparation instructions, vaccination requirements, safety instructions, or aftercare recommendations.
  • Any veterinary diagnosis, treatment decision, medication, procedure, or care provided or not provided by a veterinarian or third party.

10.2 Property and Premises Responsibility

You are responsible for the condition and safety of the service location. Smile4Pet is not responsible for damage
caused by your pet, other pets, children, household members, unsafe premises, unsecured items, inadequate working
space, or conditions outside our reasonable control. You agree to secure valuables, fragile items, other animals,
and any hazards before the appointment begins.

10.3 Maximum Liability

To the fullest extent permitted by applicable law, the total liability of SMILE FOR PET LLC for any claim arising
out of or related to these Terms or our services shall not exceed the total amount paid by the Client for
the specific appointment
giving rise to the claim.

10.4 Indemnification

You agree to indemnify, defend, and hold harmless SMILE FOR PET LLC and its members, employees, technicians,
agents, contractors, and partner facilities from any claim, demand, loss, liability, damage, cost, or expense
(including reasonable attorneys’ fees) arising out of your breach of these Terms, inaccurate or incomplete
information you provided, lack of authority to book the appointment, your pet’s behavior, unsafe premises,
unpaid charges, or claims brought by another owner or third party connected to the pet or appointment.



11. Business Practices and Compliance

11.1 Compliance With Applicable Requirements

Smile4Pet makes reasonable efforts to operate in compliance with applicable federal, state, and local laws,
regulations, and consumer-protection requirements in the jurisdictions where we provide services. Because legal,
regulatory, and local requirements may change, Smile4Pet reserves the right to modify, suspend, decline, or limit
services in any area as needed.

11.2 No Misleading Claims

Smile4Pet does not represent its anesthesia-free oral hygiene service as veterinary dental treatment. We make
reasonable efforts to explain the scope and limits of our service, disclose fees before they are applied, and avoid
misleading or unfair business practices.



12. Electronic Acceptance

By checking a box, clicking a button, submitting a form, sending a booking request, confirming an appointment,
or otherwise indicating acceptance electronically, you agree that your electronic acceptance has the same legal
effect as a handwritten signature to the fullest extent permitted by law.



13. Changes to These Terms

We reserve the right to modify these Terms at any time. The most current version will be published at

smile4pet.com/appointment-booking-terms-and-conditions/
or another page on our website designated for booking terms. Changes take effect upon posting unless
otherwise stated. Continued use of our booking service after a change is posted constitutes acceptance of the
updated Terms.



14. Governing Law and Disputes

These Terms are governed by and construed in accordance with the laws of the State of Connecticut,
without regard to its conflict-of-laws principles, except where another jurisdiction’s law applies by mandatory rule.

and give us a reasonable opportunity to resolve the matter directly.

To the extent permitted by law, any dispute arising out of or relating to these Terms, your booking, or our services
shall be resolved in the state or federal courts located in Connecticut, unless Smile4Pet elects another legally
available forum or dispute-resolution method.



15. General Terms

15.1 Entire Agreement

These Terms, together with any written appointment confirmation, written pricing confirmation, consent form,
intake form, and written service-specific instructions provided by Smile4Pet, constitute the agreement between
you and Smile4Pet regarding the appointment. Informal statements, website summaries, social media posts, prior
messages, or verbal discussions do not modify these Terms unless Smile4Pet confirms the modification in writing.

15.2 Severability

If any part of these Terms is found to be invalid, unlawful, or unenforceable, the remaining parts will remain in
effect to the fullest extent permitted by law.

15.3 No Waiver

Smile4Pet’s failure to enforce any provision of these Terms does not waive our right to enforce that provision or
any other provision later.
